Next, please download, print, and fill out the Diocesan Parent Consent form:
Next, please download, print and fill out the Diocesan Parent Code of Conduct Form:
Parent Code of Conduct form. Please complete one form per family per sport.
PLEASE include your registration fees with the forms; the fees are as follows:
- $150 per player – max of $300 per family-please make check out to SBAA
Finally, you can drop the Diocesan Parent Consent Form, Diocesan Parent Code of Conduct Form, and the check off at the school office, or mail to :

So that is… 1 Diocesan Parent Consent form per player, 1 Diocesan Parent Code of Conduct form per family, and payment ($150 per player, $300 family max).
