The parent consent form cannot be sent electronically, so you will have to print it out after you complete it.
Please complete one form per player. Be patient, this form loads slow sometimes.
PLEASE include your registration fees (Check payable to SBAA) with the form; the fees are as follows:
- $65 per player if you want a jersey for your player
- $50 per player if you have a jersey from last year
